Your CLICK HERE link takes you to Yahoo sign in and not to a site that allows me to enter my info as indicated.

The CLICK HERE is designed to open up your default email program, pre-filled with request details. All you need to enter is your mailing address.

If yahoo is your default email program it will try to open up an email form already filled in. But if you are not logged in to yahoo, then your email program will require that you login first. If yahoo is not your default email program, then I don’t know what the problem is. I have checked and it is functional. Try logging in to yahoo and the try the CLICK HERE link.

Let me know if this does not work.
